If you are trying to readmit a former patient to the clinic you need to carry out the following steps
1. Go to the reporting section on the left-hand side of the screen and then select discharged patients from the list
2. In the discharge list to search for the correct patient press Control+F and a search box will appear for you to search the discharge list with
3. Click on the name of the patient you want to readmit and then on the right-hand side select the readmit patient button and confirm to readmit
4. You should ask the patient to log in again on the app and it should start working from there. If the patient cannot log in you can ask them to reset their password using our password reset site (https://id.livingwith.health/reset-password-request). It is also advisable to ask them to check what email address they were originally enrolled with as this has to be used to reset the password.
